There's no medieval based game that is so open as Elder Scrolls. For an experience that's more closed off, but still fun, you could try "Dragon Age: Origins." If you like that you can try the inferior Dragon Age 2.Any games like Elder Scrolls?
There's a good chance you'd have to go the MMO route to get the whole sword and spell expansive fantasy world.
Not really a shortage of free to play ones if you go that way like

and I think Everquest2 is going free to play soon aswell.
Nothing I've found really compares to the fallout and elder scroll series for roaming, fighting and random exploring for raw scale. It's a lot easier to find a competitor for single aspects of the games like combat, exploring, questing... the ones you mentioned do all of it at the same time better than any alternatives really.
