I'm looking to get this as a gift, but I'm pretty sure the person I'm getting it for hasn't played any other Elder Scrolls.Do you need to be familiar with the first four Elder Scrolls before playing Skyrim?
Not at all. Skyrim takes place 200 years after Oblivion, the last Elder Scrolls game. Be warned though. Some side effects of Skyrim include:

Loss of sleep

Fatness

Never getting laid

Never seeing him again

Slipping grades

A loss of caring for anything else that isn't Skyrim.

Besides those, you're good.
Not really, since skyrim takes place 200 years after the 4th elder scrolls game there isn't much story linked to the other games. You don't need to know the previous titles. Nevertheless, I would recommend playing Oblivion anyway, just because it's also a ton of fun.

No, but I would recommend getting him Oblivion and Morrowind also.
